Our Benefit Plan was created specifically for our uninsured patients. For an annual fee, most preventative services will be covered at no additional cost to the patient. If a patient chooses to renew the plan, they will simply pay the annual fee and continue to receive the same benefits! Additional family members will be offered discounted rates to sign up.

  • · 2 Routine Cleanings (Prophylaxis and/or Perio Maintenance)
    · 2 Routine exams (Comprehensive and/or Periodic)
    · 1 Problem Focused Exam
    · All X-Rays deemed necessary for treatments
    · 1 Fluoride Application

    20% off all other services in our office with the exceptions of Ortho & Whitening

    • $300 for the first member

    • $250 for the second member

    • $200 for every member after

    The fee must be paid in full before benefits can be applied. Each member will have benefits for exactly one year from time of signing up. Members of a plan do not all have to sign up at the same time.

    • Who administers this plan?
      This plan is 100% administered in-house. This will create hassle-free experiences for the patient and us! As such, there are no waiting periods, no deductibles, and no claim forms!

    • What if I need a deep cleaning?
      Scaling and root planing is not included under the plan's routine cleaning benefit; however, the 20% discount does apply. After treatment, the plan covers 2 of the 4 recommended periodontal maintenance visits each year. The remaining 2 visits are the patient's responsibility and will receive the 20% plan discount.

    • What if I need dentures?
      While you may not gain the benefit of having semi-annual cleanings, you would still gain the 20% discount for the denture(s) and any needed adjustments. We also recommend seeing patients with dentures at least once per year for an oral cancer screening.

  • The Benefit Plan is only available to uninsured patients.

    While we do accept CareCredit for most procedures, it cannot be used to cover the Annual Fee.

    Periodontal Services are not covered under preventative services, but discounts will still apply.

    Ortho Treatment (Invisalign) & Whitening services do not qualify for the 20% discount.
